Meeting Street is currently looking for a Health Coordinator for 30 hours per week.
Duties:- Conduct home visits to assess and monitor the health, growth, and development of enrolled children.
- Provide health education to families, focusing on preventative care and addressing specific health concerns.
- Make referrals to healthcare providers and community resources as needed and follow up to ensure services are received.
- Maintain accurate and up-to-date health records for all enrolled children, ensuring confidentiality and compliance with program standards.
- Enter and track health data in systems such as Kids Net and Child Plus.
- Assist with the preparation of required reports, including the Program Information Report (PIR).
- Participate in case conferences and multidisciplinary team meetings to coordinate care for families.
- Work with staff to identify and address health-related needs impacting children’s development.

Requirements- A minimum of 1 year of experience working with infants, toddlers and families in an Early Childhood setting.
- Home visiting and or case management experience within a multidisciplinary team preferred.
- Licensed as a Registered Nurse by the Board of Nurse Registration and Nursing Education – Rhode Island Department of Health.
- CPR and first aid certifications or be willing and able to become certified.
- Early Intervention service is provided in the child’s Natural Environments, which include the home, childcare settings and the community or other immediate environments natural the child and family’s environment.
- Employees must have access to their own vehicle, with valid insurance and a valid driver’s license.
- Meeting Street will provide an agency smartphone to maintain communication with Clients and other Meeting Street employees.
- Able to get at child’s level, climb stairs, deal with weather conditions, lifting, bending, stooping.
