The Providence Center Case Manager ACT II Team 1E coordinates services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms. Encourage consumers to attain the highest possible levels of independence.
The role requires a degree in Human Services and experience in the field, with a bilingual Spanish preference. It includes in-person client contacts, home visits, and collaboration with a multidisciplinary team.
