- Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- One year of experience as a Physical Therapist
- Hourly: $76.00 - $106.00 per visit/unit
- Annual: $111,600 - $153,600
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.
This role is part of Humana's driver safety program and therefore requires a valid state driver's license and personal vehicle liability insurance with minimum limits of $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 property damage or whichever is higher.
