Position

Purpose:

Provides certified nursing assistant services to assigned patients in accordance with care plans, facility policies and procedures and at the direction of supervisor(s).

This job description is intended to convey the general scope of the major duties and responsibilities inherent in this position. Other tasks not listed here may be assigned if the tasks are similar or related to the essential duties of the position. Periodic revision may be necessary to reflect changes in expectations placed on the long‑term care industry by various governmental agencies.

This job description will be reviewed and/or revised annually and as needed.

• Certified Nursing Assistant in good standing with the state (preferred, not required)
• High school diploma or equivalent
• Current CPR/BLS certification
• Current Food Handler Permit (when applicable)
• No disciplinary action in effect against certification
• At least one year of experience as a certified nursing assistant in a SNF or ALF (preferred, not required)

• Provides supportive services to nurse, med tech and other staff as needed and performs duties as assigned
• Assists residents with or performs activities of daily living in accordance with care plans and established policies and procedures
• Assists residents with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinates dining room services at assigned mealtimes, including set‑up and clean‑up, meal tray delivery, feeding assistance, and documentation of meal intake
• Delivers nutritional supplements to patients at assigned times and provides assistance as necessary to ensure intake, documenting intake accordingly
• Assists with weighing patients according to facility policy, records weight in designated locations, and reports weight changes to nurse and supervisor
• Assists with tracking the condition of a resident’s skin, reporting any presence of pressure areas, skin breakdown or skin tears to nurse and supervisor
• Assists nursing staff in carrying out toileting program activities
• Completes flow sheets daily to indicate that the specified task was done, documenting the progress of each resident as directed by supervisor
• Keeps nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (e.g., resident refusal, reassignment to nursing unit)
• Reports changes in a resident’s condition to the resident’s nurse and supervisor

• Treat all patients with dignity and respect; promote and protect all patients’ rights
• Establish a culture of compliance by adhering to all facility policies and procedures, complying with standards of business conduct, and state/federal regulations and guidelines
• Follow appropriate safety and hygiene measures at all times to protect patients and themselves
• Maintain confidentiality of protected health information, including verbal, written, and electronic communications
• Report noncompliance with policies, procedures, regulations or breaches in confidentiality to appropriate personnel; report any retaliation or discrimination to HR or compliance officer
• Report any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of patients to supervisor and/or administrator; protect patients from abuse and cooperate with all investigations
• Report any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy
• Participate in all life safety and emergency drills and trainings; fulfill responsibilities as assigned during activation of the facility’s emergency plan
• Report work‑related injuries and illnesses immediately to supervisor
• Follow established infection control policies and procedures
• Complete all assigned training and skills competency as a condition of employment
