Home Health Case Manager
Registered Nurse will serve as Home Health Case Manager, coordinating skilled nursing care and collaborating with physicians, therapists, and other professionals to deliver patient-centered care in home settings. The role emphasizes clinical judgment, independent caseload management, and adherence to regulatory standards.
Key Responsibilities:- Conduct comprehensive home assessments to determine patient needs
- Develop and update individualized care plans
- Coordinate care with physicians, therapists, social workers, and other professionals
- Provide skilled nursing services including wound care, medication management, disease education, and chronic condition monitoring
- Educate patients and families on diagnoses, treatments, medications, and self-care
- Monitor patient progress and adjust care plans as needed
- Complete accurate and timely electronic documentation
- Ensure compliance with Medicare, state, and agency regulations
- Participate in case conferences and interdisciplinary team meetings
- Advocate for patients to ensure highest quality of home healthcare services
- Current Registered Nurse (RN) license in the State of Utah or eligibility for licensure
- Graduate of an accredited nursing program
- Minimum of one year of RN clinical experience
- Strong assessment, critical thinking, and clinical decision-making skills
- Excellent communication and interpersonal abilities
- Comfortable working independently and managing daily schedules
- Valid driver's license with reliable transportation
- Current auto insurance meeting state requirements
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program
- Active Utah Registered Nurse (RN) license
- Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross
Skills:
- Previous home health, hospice, case management, or community health experience
- Experience with electronic medical records (EMR)
