We are seeking an experienced and passionate Nurse Case Manager to help implement Family Caregiver Services for our children and families.

Role Overview

The Nurse Care Manager is a highly qualified health care provider who applies the art and science of nursing to assess, implement, and evaluate client responses to health problems. The role involves direct nursing care as prescribed by physicians to acutely or chronically ill clients in their homes, recording clinical and progress notes, coordinating other care activities, and overseeing client care in the geographic service area assigned.

The Nurse Case Manager completes Pediatric Assessment Tools (PAT), manages clinical duties, generates plans of care, and assists with clinical operations and visits. Administrative tasks include service coordination, documentation requests, and record review. The position requires proficiency with EMR systems and multiple technological platforms.

Key Responsibilities
  • Effectively present information to clients, employees, and the community.
  • Respond to sensitive inquiries or complaints.
  • Assess, define problems, collect data, establish facts, and draw valid conclusions.
  • Handle abstract and concrete variables.
  • Improvise in a home setting.
  • Send/receive emails and use the internet for research and informational purposes.
  • Use MS Excel and MS Word for documentation.
  • Use independent judgment to manage confidential information.
  • Attend to multiple time-sensitive tasks simultaneously.
  • Apply knowledge of speech therapy principles and scope.
  • Maintain thorough knowledge of speech therapy theory, home health care practice, communication skills, and case management.
  • Demonstrate basic knowledge of Windows OS and advance computer skills as required.
  • Establish and maintain effective working relationships with employees, the public, and supervisors.
  • Function in any home situation regardless of client demographics.
  • Work harmoniously with a wide variety of people and all levels of staff.
  • Show strong commitment to the organization’s goals, mission, and philosophy.
  • Work flexible hours to meet client needs.
  • Show integrity and sound judgment in handling confidential assignments.
  • Adapt to changing organizational needs.
Qualifications
  • Bachelor’s Degree in Nursing and at least one year of experience in home care specifically surrounding the CNA Program.
  • Licensed as a RN in the State of Colorado.
  • BLS certification strongly recommended.

Compensation

Annual Salary Range: $80,000 to $85,000/year
