LPN Licensed Practical Nurse

University Park Care Center in Colorado Springs, CO is seeking a Licensed Practical Nurse to provide compassionate, resident-centered care in our skilled nursing and rehabilitation facility. The LPN will deliver direct bedside care, administer medications and treatments, monitor and document resident conditions, and support recovery and long-term care goals. You'll collaborate closely with RNs, therapists, physicians, and CNAs to implement individualized care plans, educate residents and families, and promote safety, dignity, and comfort.

Responsibilities

• Provide direct bedside nursing care to residents
• Administer medications and treatments per physician orders
• Monitor, assess, and document resident conditions and vital signs
• Implement and support individualized care plans
• Collaborate with RNs, therapists, physicians, and CNAs
• Support rehabilitation and long-term care goals
• Educate residents and families on care and health needs
• Promote resident safety, comfort, dignity, and quality of life
• Report changes in condition promptly to supervising nurse
• Maintain accurate, timely clinical documentation
