Gentiva is seeking a Nurse Practitioner to provide comprehensive hospice and palliative care to patients in various settings. The NP will conduct management visits, participate in Face-to-Face encounters for eligibility certification and recertification, and collaborate with physicians and the IDG/IDT to ensure high-quality, compliant care.
Ideal candidates have an MSN or higher, completion of an NP program, and NP certification with state licensure. Hospice/palliative experience is preferred.
