Join our caring community as a Home Health Registered Nurse, reporting to the Clinical Manager, providing personalized care in patients' homes.
Key responsibilities include:- Administering medications, wound care, and conducting assessments onsite
- Developing and implementing individualized care plans collaboratively
- Monitoring patient conditions and reporting changes
- Educating patients and families on disease management and treatments
- Maintaining accurate documentation and coordinating with healthcare teams
- Overseeing Home Health Aides and ensuring proper documentation
- Med surg, ICU, ER, or acute care experience
- Active Colorado RN license and current CPR certification
- Team collaboration skills
- Valid driver’s license, auto insurance, and reliable transportation
- One year nursing experience, including home health
Additional information: Role involves patient-facing duties, TB screening, and adherence to driver safety policies.
