We are currently seeking a dynamic individual to serve as the Complex Case Manager for Level 1 or 2 for the Lead Program.
Please note that all applicants must undergo a pre-employment drug screening, which includes testing for marijuana, as well as a comprehensive background check and TB Test.
Job OverviewPREVAIL (Pre-Arrest Evaluation, Intervention, and Linkage) helps people avoid arrest, court, and jail when their negative Juvenile behavior is driven by substance use, behavioral health, or unmet basic needs. Instead of punishment, PREVAIL connects people to the services that address the root problem.
PREVAIL serves people age 15 and up:
- Adults (18+) are usually referred by police but can also be referred without an offense.
- Juveniles (15–17) are usually referred by municipal court, schools, school SRO officers, or other community partners.
- Referrals can also come as social referrals through community stakeholders.
The Complex Case Manager helps participants meet basic needs — food, clothing, shelter — and connects them to community resources. This support helps reduce reoffending and helps people build a stable foundation.
The Complex Case Manager stays involved from the moment of referral through ongoing case management, meeting participants wherever they're comfortable. Staff must pass an extensive background check due to contact with juveniles.
Position Responsibilities Diversion and Referral CoordinationAdult Participants
- Accept participants from police custody, diverting them from arrest and charges into services.
- Partner with law enforcement to connect people to help instead of jail.
- Serve as the main point of contact between law enforcement, participants, and service providers.
- Accept social referrals from community partners, in addition to law enforcement.
Juvenile Participants
- Accept referrals from municipal court, schools, school resource officers, and community stakeholders.
- Work with the courts during the diversion process — attend hearings and share progress updates with a current signed Release of Information (ROI).
- Accept social referrals from community partners.
- Partner with schools and youth organizations to identify young people who could benefit from diversion.
- Serve as the main point of contact between courts, schools, families, participants, and service providers.
- Provide basic needs like food, clothing, and shelter.
- Connect participants to housing, treatment, benefits, and other resources — for youth, this includes helping families and schools too.
- Help participants stay engaged in services and, for youth, stay in school.
- Meet participants in the field — public places, parks, or schools for youth.
- Build trust by meeting people where they are, physically and emotionally.
- Balance office and community work.
- Be available for on-call coverage as needed.
- Work a flexible schedule, including days and evenings, in the office and community.
- Take on other duties as needed to support the PREVAIL Program.
All Complex Case Managers receive training on:
- PREVAIL's model and pre-arrest diversion principles.
- Case management basics, including resource navigation and warm handoffs.
- Field-based and school-based engagement and safety practices.
- De-escalation and motivational interviewing.
- Documentation and the electronic health records (EHR) system.
