Position Purpose

The Veteran Support Specialist provides rental assistance to very low-income and extremely low-income Veteran households enrolled in SSVF’s Rapid Rehousing or Homeless Prevention services who need longer-term financial assistance to maintain safe, stable housing. The role includes cost-shared rental assistance to landlords for up to two years and, when needed, limited progressive case management services to help Veteran households maintain housing stability.

The VSS delivers trauma-informed case management and works as a member of a team to provide supportive services while meeting program requirements under the SSVF program. The position may require occasional travel throughout Colorado and involves developing relationships with community partners and housing providers. Detailed documentation of client case files is required to meet VA and RMHS standards.

Essential Duties
  • Provide progressive case management services to the Veteran client (and household members) through office meetings, home visits, and client transportation to access housing opportunities and community services (driving a company vehicle).
  • Maintain regular contact with clients at least monthly to monitor progress on rental payment independence.
  • Maintain accurate, detailed documentation of client case files and input required data into the Caselogic system and the VA HMIS; meet program documentation milestones.
  • Document progress, linkages, and referrals to community-based resources, including income supports.
  • Develop a Housing Stabilization Plan, assess goals and needs, and assist clients in selecting among available service providers, considering preferences and factors such as gender, age, culture, language, location, and hours of availability.
  • Collaborate with Veterans on discharge planning to maintain permanent housing.
  • Develop and maintain knowledge of governmental and community programs to support housing stability; research additional funding sources as needed.
  • Maintain working knowledge of SSVF program guidelines and ensure adherence when providing services.
  • Conduct problem-solving conversations to identify resources and alternative housing options, building rapport to resolve homelessness.
