Licensed Practical Nurse (LPN) – Family Health Clinic

Job Type: Full‑Time – Days

Schedule: Monday‑Thursday 8a‑5p;
Friday 8a‑12p

• Provide direct patient care under the supervision and delegation of a registered nurse, following scope and standards of practice.
• Perform plan‑of‑care interventions, including specimen collection, wound care, and medication administration; document all patient care.
• Assist the RN in developing and implementing plans of care.
• Support patients with activities of daily living: personal hygiene, elimination, nutrition, and ambulation.
• Monitor, document, and communicate patient status and responses to interventions with the RN, provider, patient, and families.
• Advocate for the physical and emotional well‑being of patients.
• Schedule appointments and procedures as ordered by the provider.
• Obtain insurance re‑authorizations for testing and scheduled procedures.

• Current Tennessee or compact‑state LPN license.
• Graduate of a Practical Nursing program.
• Required certification: AHA Basic Life Support (BLS) within 30 days of hire.
• Critical thinking, decisive judgment, and the ability to work with minimal supervision.
• Capacity to work in a stressful environment and take appropriate action.
• Proficiency in communicating effectively with patients, families, and healthcare team members.
