JOB STATUS: Non-Exempt

OFFERED SALARY RANGE: $21.65 - $23.42 per/hr.

FULL SALARY RANGE: $21.65 - $57.73 per/hr. (Represents the low and high end of the total salary range for this position)

The full salary range representing the broader compensation band for the position and progression within that range is based on employee longevity and continued service. Summary

We’re pleased to announce an opportunity for the position of Case Manager within the Senior and Independent Living Services Department.

Position Summary

Provides comprehensive case management services to individuals or families on assigned Senior & Independent Living Services program(s). Assesses needs, develops and monitors implementation of service plans, makes appropriate referrals and serves as a client advocate with other service providers. Responsible for executing specific program standards and delivering or facilitating the delivery of program related services. Requires knowledge of related community resources and the laws and regulations specific to the area of service.

Essential Job Functions
  • Complete the initial interviews with clients and their families to assess the customer’s eligibility for services.
  • Gather relevant and useful data from the client, family, other agencies, to formulate a psychosocial assessment of the client and his or her family.
  • Assemble and guide group discussions and decision-making sessions among relevant professionals and program representatives, clients and their families, and significant others to formulate goals and design an integrated intervention plan.
  • Act as a liaison between the client and his or her family and all relevant professionals, programs, and informal resources involved in the overall intervention plan to help the client make his or her preferences known and secure the services needed.
  • Complete the necessary paperwork to maintain documentation of client progress and adherence to the plan by all concerned.
Minimum Qualifications

Required Education or

Experience:

  • Four years of paid, full time, on-the-job social service experience in providing care management and long-term support and services to vulnerable adults, such as providing one-on-one, in-person customer service, developing care plans for aged, blind, or disabled individuals and caseloads, and a high school diploma or its equivalent; or
  • Three years of paid, full time, on-the-job social service experience, providing one-on-one, in-person customer service, developing care plans for aged, blind, or disabled individuals and caseloads, a high school diploma or its equivalent, and are bilingual or bicultural with such experience; (or
  • Two years of experience with other DSHS information systems used by Case Management staff, such as Comprehensive Assessment and Reporting Evaluation (CARE) tool, Barcode, Provider One, QA Monitor, ACES.Online, or ALTSA reporting, and college courses, an Associate’s degree, or its equivalent; or
  • Two years of paid, full time, on-the-job social service experience and Bachelor’s degree; or
  • One year of paid, full-time, on-the-job social service experience and a Bachelor’s degree in social services, behavioral or health sciences; or
  • Social service experience and a Master’s degree.
