The Health Home Care Coordinator provides comprehensive care coordination services to eligible individuals and their families. This role involves assessing member needs, developing and monitoring individualized service plans, making appropriate referrals, and advocating on behalf of members with other service providers. Care Coordinators maintain a dedicated caseload and ensure consistent monthly engagement with assigned members across various settings. Health Home Care Coordinator's support members in identifying and accessing resources, delivering health education, and applying motivational interviewing techniques to foster goal achievement, resilience, and healthy lifestyle choices.
The Care Coordinator promotes wellness through coaching and awareness of chronic health conditions, aiming to reduce emergency service usage and prevent hospital readmissions.
