Job Description

The Certified Nurse Assistant provides nursing and nursing-related services to residents consistent with each resident’s comprehensive resident assessment and plan of care. All resident care is provided in a manner that meets the resident’s physical, mental and psychosocial needs and enables the individual to attain or maintain the highest practicable level of functioning. The Certified Nurse Assistant maintains a “homelike” environment for the residents, protects and promotes resident rights and assists the person to maintain independence and control to the greatest extent possible.

Responsibilities
  • Provide direct care to residents to improve or maintain the resident’s abilities according to the resident’s plan of care.
  • Assist with activities of daily living (ADLs) including bathing, grooming, oral care, dressing, mobility, transferring, and feeding of assigned residents. This includes preparing residents for bathing, assisting with dressing, maintaining privacy, and ensuring proper clothing is used.
  • Assist with restorative care to maintain functional status, including ambulation, range of motion exercises, use of assistive devices, and maintaining proper body alignment to prevent contractures.
  • Assist with elimination and toileting, including patient transport to bathroom, bedpan or urinal, cleaning, catheter care and data recording as directed.
  • Assist with mobility, including weight‑bearing assistance, use of wheelchairs, and environmental safety checks.
  • Assist with transfers, positioning residents and using proper techniques and equipment as per the plan of care.
  • Assist with eating, providing meal setup, feeding for those requiring total assistance, encouraging proper eating and documenting intake.
  • Protect and promote resident rights, dignity, self‑determination, active participation, and respect for resident choices and property.
  • Observe and report patterns of decreased social interaction, emotional behaviors, and refusals of treatment to the Unit Charge Nurse.
  • Assist in planning, implementation and evaluation of plans of care; report conditions, responses and suggestions for alternative approaches.
  • Assist with preparation for state inspections, attend survey training and interact with state surveyors as directed.
  • Follow all facility policies and procedures, including infection control and use of personal protective equipment.
  • Record accurate, legible information about resident care and condition in resident records.
  • Maintain and demonstrate proper body mechanics; use equipment such as back belts, gait belts and mechanical lifting devices.
  • Monitor physical environment for safety or fire hazards; report hazards and maintenance issues promptly.
  • Report injury to resident or self to Charge Nurse immediately.
  • Attend 12 hours of in‑service education annually and apply information to job tasks.
  • Participate in fire and disaster drills and execute assigned duties to assure resident safety.

  • Comply with workplace safety policies including infection control, standard precautions, use of PPE, and hazardous material handling.
  • Prioritize tasks and complete duties within allotted time.
