A respected community hospital is seeking a Physical Therapist (PT) to join its collaborative rehabilitation team. This position offers a unique blend of outpatient therapy and inpatient care

Hospital-based therapy program with strong interdisciplinary collaboration
Balanced caseload: outpatient ortho, post-surgical, neuro, and inpatient coverage

$15,000 sign-on bonus and relocation assistance available
Physical Therapist ResponsibilitiesEvaluate patients and develop individualized physical therapy treatment plans based on physician referrals
Provide evidence-based physical therapy interventions to restore mobility, strength, balance, and functional independence
Treat a variety of diagnoses including orthopedic, neurological, and post-surgical conditions
Educate patients and families on home exercise programs, recovery expectations, and injury prevention
Collaborate with physicians, nurses, case managers, and therapy staff to coordinate care plans
Maintain accurate EMR documentation including evaluations, progress notes, and discharge summaries
Participate in care planning meetings and quality improvement initiatives
Doctor of Physical Therapy (DPT) or equivalent from an accredited Physical Therapy program
Current Physical Therapist license or eligibility for licensure
Strong clinical reasoning and patient communication skills
Ability to work both independently and collaboratively within a multidisciplinary team
