• Perform clinical PT assessments to determine patient needs and appropriateness of therapy
• Develop, implement, and revise a Plan of Care (POC) and document progress notes, phone orders, and other clinical records
• Collaborate with an interdisciplinary team and coordinate with physicians to plan and provide treatment plans for neuromuscular, neurological, and orthopedic conditions
• Educate patients and families on transfer techniques and safe use of equipment (walkers, wheelchairs, canes, braces, prosthetic devices) and arrange necessary rehabilitative equipment
• Teach and supervise patient/family in home exercise programs and safe equipment use; adapt equipment as home environments require
• Ensure infection control and universal precautions in all patient interactions; manage clinical assignments within competence and scope
• Provide guidance to other team members as appropriate and participate in ongoing professional development

• Current Physical Therapist license in Washington state – Required
• CPR certification – Required
• Current driver's license with reliable transportation and insurance – Required
• 1–2 years of physical therapy experience in an acute care or rehabilitation setting – Required
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association–approved program – Required
• Previous home care experience – Preferred; ability to work a flexible schedule and travel locally; able to perform heavy lifting and prolonged standing
