Position OverviewWe are seeking a skilled and compassionate Registered Nurse (RN) to provide high-quality home health and hospice care to patients in their place of residence. This role focuses on delivering patient-centered care, coordinating with interdisciplinary teams, and ensuring continuity of care across all stages of treatment.
Key Responsibilities for Registered Nurse (RN) – Home Health & Hospice- Perform comprehensive patient assessments in both routine and emergent situations
- Develop, implement, and evaluate individualized Plans of Care (POC)
- Coordinate care with physicians, interdisciplinary teams, patients, and families
- Maintain accurate and timely clinical documentation for all patient interactions
- Provide patient and family education on medications, treatments, safety, and self-care
- Administer skilled nursing care, including IV therapy, wound care, and phlebotomy
- Monitor and manage pain, adjusting care plans as needed
- Supervise and provide direction to support staff (HHA/LVN/STNA)
- Ensure compliance with infection control standards and regulatory requirements
- Coordinate referrals and ensure appropriate resource utilization
- Participate in interdisciplinary team meetings and case reviews
- Manage assigned caseload to meet productivity and quality standards
- Active, unrestricted Registered Nurse (RN) license
- Minimum 1–2 years of clinical nursing experience
- At least 2+ years of home health experience
- Homecare Homebase (HCHB) experience – required
- OASIS documentation experience – required
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
- Strong clinical, organizational, and communication skills
- Bachelor of Science in Nursing (BSN)
- CHPN certification
- Experience in both home health and hospice settings
- Skilled in IV therapy, venipuncture, and medication administration
- Proficient in wound care, catheter and drain management
- Ability to assess physical, psychosocial, and environmental patient needs
- Strong ability to work independently while collaborating with a care team
- Effective time management and ability to prioritize in a fast-paced environment
