Position Overview

Clinic‑based role: 3 days per week in a Primary Care clinic and 1 day per week in the Palliative Care clinic, both located in the same building. On Primary Care days the provider serves as an extension of an established primary care physician, handling follow‑ups, acute visits, chronic condition management, and preventative care for the physician’s panel. On the Palliative Care day the provider independently manages their own panel of patients referred for palliative care services.

Essential

Functions Primary Care
  • Assist with diagnosis, treatment, and management of acute and chronic health conditions.
  • Perform preventive visits and work towards closing care gaps and achieving ACO metric goals.
  • Prescribe medications to the extent delegated and licensed.
  • Generate referrals.
  • Participate in Provider Operations Council meetings (bi‑monthly, 30 minutes each).
Palliative Care
  • Provide consultation in palliative care, symptom management, and supportive care as requested by the referring physician.
  • Conduct goals‑of‑care conversations to help patients determine treatment goals.
  • Consult with the referring physician, acute‑care team, and other involved providers to contribute to the patient’s plan of care.
  • Prescribe medications, including controlled substances, to the extent delegated and licensed.
Both Roles
  • Order diagnostics/treatments as needed.
  • Prepare and maintain accurate patient records, charts, and documents to support medical practice and reimbursement.
  • Manage patient portal and respond to messages.
Non‑Essential Functions

  • Computer skills including Word, PowerPoint, Excel, and ability to learn an electronic medical record system.
Required Education, Licensure, Certifications, & Experience
  • Licensed to practice in the state of Washington as a Clinical Nurse Practitioner.
  • Experience working in a primary care or acute care setting.
  • Knowledge of interdisciplinary team concepts.
Preferred Education, Certifications, & Experience
  • Experience with palliative care or hospice care.
  • Certified Palliative Care ARNP.

  • Occasional reaching/handling for supplies equipment.
  • Constant fingering to perform tasks with computer, writing, telephone.
  • Constant talking/hearing/seeing for patient care and administrative duties.
