Requirements
- Current state license as a Registered Nurse - Required
- Proof of current CPR certification - Required
- Current driver's license - Required
- Must have reliable transportation and insurance - Required
- 1-2 years of experience as a Registered Nurse in a clinical care setting - Required
- 2+ years of Home health experience - Required
- Homecare Homebase Software Experience - Required
- OASIS Experience - Required
- CHPN certification - Preferred
- Bachelor of Science in Nursing - Preferred
Roles & Responsibilities
- Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
- Implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and their families regarding care, diet, medications, and home safety.
- Supervise and provide clinical direction to home health aides and nursing assistants.
- Monitor assigned cases to ensure compliance with third-party payer requirements.
- Perform on-call responsibilities and provide services as assigned.
