Solaris Healthcare Charlotte Harbor is hiring prn/per diem LPN's. Preferably a degree in social work or a degree in a human services field including but not limited to sociology, gerontology, special education, rehabilitation counseling, and psychology. Will preferably have one (1) year supervised social work experience in a health care setting working directly with individuals, discharge planning preferred.

The primary purpose of your job position is to assist residents to achieve or maintain an optimal level of self care and independence through consistent, structured and controlled reteaching and retraining programs to promote increased participation in activities of daily living, in accordance with current federal, state, and local standards, guidelines and regulations, and Solaris Health Care's established policies and procedures.

  • Receives assignment from supervisor and performs duties in accordance with each resident's detailed plan of care to provide care to residents who are presently enrolled in the Restorative Nursing Program.
  • Checks residents in section regularly;
    Answers call lights promptly;
    Follows through with resident problems or needs by delivering care or reporting to Supervisor depending upon level of ability and nature of resident need.
  • Maintains knowledge of resident's condition at all times;
    Notes significant changes in the condition of residents and takes appropriate action;
    Identifies emergency situations and responds with appropriate measures.
  • Explains procedures to resident before administering.
  • Takes and records temperature, pulse, respiration, intake and output;
    Reports resident incidents to immediate supervisor;
    Communicates requests and problems to appropriate personnel;
    Takes BPs upon request by Supervisor.
  • Utilizes restorative techniques including cuing, hand over hand, prompting, guarding, etc.
  • Assists in the assessment of resident need for assistive/adaptive devices and communicates those needs to appropriate personnel.
  • Empties urinary drainage bags and records amount in record;
    Reports any irregularities to nurse.
  • Welcomes new residents and helps them adjust to new environment;
    Assists residents, family, and staff to ensure a comfortable homelike environment, i.e.: assists resident to personalize room, monitors light and sound levels, temperature, etc.
  • Establishes and maintains good public relations with residents, families, and other center guests;
    Shows empathy toward residents, as needed.
