Per Diem Requirement:Per diem employees are expected to work a minimum of32 hours per month.

As a Health Promotion Advocate, you will be a vital member of a dynamic team responsible for performing a broad range of clinical support, administrative, and educational functions in support of Project ASSERT within the Emergency Department at Day Kimball Health. Working under the general direction of the Project Leader and in collaboration with all members of the patient care team, you will help ensure the delivery of coordinated, patient-centered care.
While specific duties are outlined, the primary responsibility of this role is to integrate health promotion and screening services into the Emergency Department patient care model. The scope of practice includes, but is not limited to, diagnostic assessment, care coordination, counseling, motivational interviewing, behavior modification, mental health screening, and disposition planning. Responsibilities may be modified as patient care needs evolve
Health Promotion Advocate Responsibilities- Integrate health promotion, screening, brief intervention, and referral services into Emergency Department patient care as part of Project ASSERT.
- Conduct patient interviews and administer evidence-based screening tools to identify substance use, behavioral health, and social determinants of health needs.
- Prioritize patient assessments and interventions based on clinical indicators and referrals from Emergency Department physicians, nurses, and other care team members.
- Provide brief interventions, patient education, motivational support, and care navigation to promote engagement in treatment and community resources.
- Coordinate referrals to internal services and external community-based providers, facilitating continuity of care and follow-up services.
- Collaborate with Emergency Department clinicians, care teams, and community partners to support patient-centered care planning and communication.
- Conduct follow-up outreach to monitor patient engagement with recommended services and identify ongoing support needs.
- Maintain accurate, timely, and confidential documentation, patient records, and data entry in accordance with Project ASSERT requirements, organizational policies, and regulatory standards.
- Maintain current resource information, project materials, and referral networks to support effective service delivery.
- Demonstrate professionalism, safeguard patient confidentiality, and comply with all Day Kimball Health policies, procedures, and standards.
Education and Experience Requirements
- Valid driver’s license and proof of current liability insurance as well as reliable vehicle required
- Bachelor’s degree preferred, or equivalent combination of education and relevant experience.
- Three (3) or more years of experience in one or more of the following:
- Community or population-based health screening
- Primary care or ambulatory care settings with significant patient interaction
- Substance use treatment, intake, or behavioral health environments
- Familiarity with community-based resources preferred.
- Knowledge of substance use disorders, behavioral health screening, motivational interviewing, and harm reduction.
