CHR Community Health Resources, Inc. is seeking a Licensed Practical Nurse to join Pathways OTP in Putnam, CT. This part-time, on-site role (20 hours/week) covers Saturday mornings plus rotating weekday shifts, supporting MAT services in our Opiate Treatment Program.
You will administer MAT meds, monitor patients, maintain records in the EHR, coordinate with clinicians, and ensure regulatory compliance. CT LPN license and MAT experience preferred.
