New Life Center, Putnam, CT

CHR's New Life Center is a Residential Substance Abuse Treatment program designed to accommodate women with substance abuse and co-occurring mental health disorders who have infants, toddlers under the age of 4 and/or expectant mothers. Children reside in the facility with their mothers in order to promote a healthy parental bond with the focus of developing effective parenting skills. Estimated length of stay is 4-6 months with the goal of preparing for a healthy and safe substance‑free lifestyle.

DCF‑involved mothers are welcomed. Mothers and infants receive medical care at Day Kimball Hospital.

• Maintains physical environment and program standards set by funders, accreditors, and agency policies.
• Monitors self‑administration of client medications in accordance with agency medication policies and procedures.
• Effectively utilizes appropriate communication methods/tools to collaborate with co‑workers and clients.
• Teaches and assists all residents in developmentally appropriate activities of daily living.
• Assists/Manages administrative functions and tasks at the direction of the supervisor.
• Prepares and facilitates educational and skill‑building groups and one‑to‑one interventions as directed by the treatment plan.
• Collaborates and coordinates care with providers and families as needed.
• Assists residents in addressing medical and psychiatric needs.
• Maintains documentation in compliance with agency and program standards.
• Participates in staff meetings and supervision.

• Education:
  
  high diploma or GED required.
• Experience:
  
  two years’ experience working with a relevant population preferred.
• Licensure/Certification/Registration: valid driver’s license required with a good driving record.
