Connected Health is seeking a Part-time School Social Worker on a 36-week contract in Putnam, Connecticut to deliver social work services to high school students, collaborate with school staff, and maintain confidential case documentation.
You will work with grades 9-12, partner with teachers and families to support student needs, assess situations, refer to community resources, and participate in team meetings and district behavioral and mental health initiatives.
