Roles & Responsibilities Evaluate patients and develop individualized physical therapy treatment plans to improve function
Deliver evidence-based therapy interventions to enhance mobility, strength, balance and independence
Complete evaluations, progress notes and discharge documentation in compliance with facility and regulatory standards
Collaborate with interdisciplinary team members to coordinate high-quality patient care
Educate patients and families on therapy goals, safety and home exercise programs to support recovery
Ensure adherence to state, federal and facility policies to maintain safe, compliant care
Qualifications Active Washington State Physical Therapist license in good standing (required)
Graduate of an accredited Physical Therapy program
Skilled nursing facility or post-acute care experience preferred
Strong clinical judgment with excellent communication and documentation skills
Ability to work independently and as part of a multidisciplinary team
Education Graduate of an accredited Physical Therapy program
Knowledge & Skills Patient evaluation and functional assessment
Individualized treatment planning and goal setting
Evidence-based therapeutic interventions for mobility and balance
Accurate clinical documentation and regulatory compliance
Interdisciplinary collaboration and patient/family education
Strong verbal and written communication skills
