Responsibilities- Foster a non-clinical, resident-centered approach to the planning and delivery of services, supporting residents’ rights to participate in decision-making and treating residents with dignity and respect.
- Assess health, functional and psychosocial status of residents; initiate individualized service plans and proactively manage care to maximize residents’ ability to remain in the assisted living environment.
- Coordinate with other community professionals (physicians, discharge planners, nurses, home health staff, case managers, state personnel) to ensure residents’ care needs are met; arrange ancillary services (pharmacy, podiatry, lab, x-ray) as needed.
- Delegate nursing tasks in accordance with applicable laws and communicate regularly with residents, families, and health care practitioners.
- Seek additional expertise to ensure residents’ needs are met and proper practices are followed.
- Provide training, supervision, and monitoring of staff delivering personal care, medication management, and other health-related services; ensure adherence to the philosophy of care.
- Communicate regularly with residents’ families about health care needs; provide instruction and reassurance for residents and families in managing health conditions.
- Collaborate with the Director of Nursing to address urgent problems and support decision-making when health concerns arise.
- Ensure resident care documentation is complete, timely, and maintained per company policies.
- Maintain up-to-date knowledge of relevant rules and regulations; apply new knowledge to meet evolving resident needs.
- Participate in marketing efforts by representing the residence positively to external agencies and in community events.
- LPN or RN license required.
- Ability to creatively solve problems in resident care and staff management.
- Minimum of 2 years in geriatric nursing, including experience with medication administration and pharmacy processes.
- One year in supervisory/management role with experience in budgets, staff development, training, and scheduling. Management coursework is an asset.
- Basic computer knowledge with ability to learn company applications.
- Current First Aid and CPR.
- Criminal background and fingerprint check clear.
- Compassion and interest in working with seniors.
- Ability to work a flexible schedule, including weekends, holidays, and overtime as required.
- Strong interpersonal skills to relate positively with residents, families, community members, volunteers, and colleagues.
- Maintain a clean, safe, and comfortable environment for residents, staff, and visitors.
- Proficient in reading, writing, speaking, and understanding English.
- Meet all mandatory health and education requirements by state regulations.
- Ability to work independently with minimal supervision.
