MDS Coordinator – RN or LPN

The Springs at Rochester Hills is seeking an experienced and detail-oriented MDS Coordinator (RN or LPN) to join our skilled nursing team. This is an excellent opportunity for a knowledgeable nurse who understands the MDS process and is committed to accurate assessments, regulatory compliance, and exceptional resident care.

Position Summary

The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) and MDS process in accordance with federal and state regulations. This position works closely with nursing, therapy, dietary, social services, and other members of the interdisciplinary team to ensure assessments and care plans accurately reflect each resident’s needs.

Key Responsibilities
  • Coordinate and complete accurate, timely MDS assessments

  • Manage the RAI process and assessment calendar

  • Lead interdisciplinary care plan meetings

  • Ensure care plans reflect residents’ current needs, goals, and services

  • Review clinical documentation for accuracy, completeness, and regulatory compliance

  • Monitor changes in resident condition and ensure appropriate assessments are completed

  • Collaborate with nursing, therapy, social services, dietary, and other departments

  • Support accurate reimbursement through proper assessment and documentation

  • Stay current with CMS guidelines, Medicare requirements, and MDS updates

  • Assist with audits, surveys, and quality-improvement initiatives

Required Qualifications
  • Current, active Michigan RN or LPN license

  • Previous MDS experience in a skilled nursing facility is required

  • Working knowledge of the RAI process, care planning, and skilled nursing regulations

  • Strong clinical assessment and documentation skills

  • Excellent organization, communication, and time-management abilities

  • Ability to manage deadlines and work effectively with an interdisciplinary team

  • Experience with Medicare, Medicaid, and reimbursement guidelines is preferred
