Job Description:
The Social Worker
- Social Worker Case Manager is required to have at least 2 years of recent acute inpatient social work/case management experience and 4 years of overall social work experience. The candidate must be skilled in acute care case management, behavioral health assessment, diagnosis, treatment planning, and discharge planning for post-acute placements.
Responsibilities:
- Provide case management for inpatients, focusing on adults in ICU, PCU, Med Surg, and ER settings.
- Conduct behavioral health assessments, create treatment plans, and plan discharges.
- Serve as a remote resource for off-campus EDs when needed.
Requirements:
- Master's in Social Work and Washington state license.
- Licensure requirements vary by assignment; LICSW required for ER, LSWAIC minimum for inpatient.
- EPIC experience is necessary.
