Banner Health seeks an LPN/LVN to provide nursing care in a community setting at our Queen Creek clinic. The role reports to a registered nurse and/or licensed physician and is accountable for the quality of nursing services delivered.

• Observe, compile and report patient’s physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs; review information from health‑care team members.
• Contribute to plan of care under direction of a registered nurse, including discharge planning; collaborate with the multidisciplinary team and update plans as patient needs change.
• Implement delegated interventions, patient care, and nursing plan of care within scope of practice; ensure compliance with policies, procedures, standards of care, and regulatory agencies.
• Report changes in patient condition, progress toward goals, and teaching effectiveness to the registered nurse and/or physician.
• Document observations, implementation, and data in the patient record; note physician orders accurately and promptly.
• Plan, organize, develop, and coordinate documentation, ensuring compliance with federal and state regulations; audit reimbursement documentation and quality management processes.
• Coordinate admission processes, including new resident evaluations, and communicate with residents, families, referral sources, insurance companies, and clinical staff.
• Work under supervision, prioritizing data from multiple sources to provide quality care; interact with staff at all levels as well as patients, families, and physicians.

• Valid LPN or LVN license in the state of practice (including temporary or compact license).
• BLS certification required; additional certifications or continuing education may be required.
• Completion of a state‑approved IV therapy program may be required for certain facilities.
• For Banner Registry and Travel positions, at least one year of acute‑care experience as an LPN or LVN within the past 12 months.
• Employees in Banner Home Care AZ must possess and maintain an Arizona Fingerprint Clearance Card.

• Experience in the designated clinical area.
• Additional related education and/or experience preferred.
