Position: Licensed Practical Nurse (LPN) - DSL
Position Summary

The Licensed Practical Nurse (LPN) provides direct nursing care under the supervision of a Registered Nurse (RN) or Nursing Supervisor. The LPN is responsible for administering medications and treatments, monitoring resident health, documenting care, and supporting residents' physical and emotional well-being while ensuring compliance with all federal, state, and facility regulations.

Essential Responsibilities
  • Provide direct nursing care to residents in accordance with individualized care plans.

  • Administer prescribed medications, injections, treatments, and wound care accurately and safely.

  • Monitor residents for changes in condition and promptly report findings to the Registered Nurse or physician.

  • Complete accurate and timely documentation in the electronic medical record (EMR).

  • Assist with admissions, transfers, and discharges.

  • Supervise Certified Nursing Assistants (CNAs) and delegate resident care assignments appropriately.

  • Ensure resident safety and maintain infection prevention and control standards.

  • Respond promptly to resident needs, emergencies, and changes in medical condition.

  • Communicate effectively with residents, families, physicians, and the interdisciplinary care team.

  • Participate in care planning meetings and quality improvement initiatives.

  • Maintain compliance with New York State Department of Health regulations, CMS guidelines, and facility policies.

Qualifications

Required:

  • Current New York State Licensed Practical Nurse (LPN) License.

  • Graduate of an accredited Practical Nursing Program.

  • Current CPR/BLS Certification.

  • Strong communication, organizational, and interpersonal skills.

  • Ability to work effectively in a fast-paced skilled nursing environment.

Preferred:

  • Previous experience in long-term care, rehabilitation, or skilled nursing.

  • Experience with electronic medical records (EMR).

  • Knowledge of infection prevention, resident rights, and CMS regulations.
