Registered Nurse

Glens Falls Center seeks a compassionate Registered Nurse to provide high-quality clinical care to residents in our skilled nursing and rehabilitation facility. The RN will complete assessments, administer medications and treatments, monitor resident conditions, and collaborate with an interdisciplinary team to develop and implement individualized care plans. Responsibilities include supervising CNAs, documenting in the EMR, educating residents and families, and promoting safety and infection control.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing and rehab setting
• Perform comprehensive assessments and ongoing monitoring of resident conditions
• Administer medications, treatments, and therapies per provider orders
• Develop, implement, and update individualized care plans with the care team
• Supervise, mentor, and support CNAs and other support staff
• Document accurately in the EMR and maintain regulatory compliance
• Communicate with residents, families, and providers regarding care and progress
• Promote resident safety, infection control, and quality standards
• Participate in interdisciplinary care conferences and quality improvement initiatives
• Educate residents and families on health conditions, medications, and care plans
