Licensed Practical Nurse

Glens Falls Center seeks a compassionate Licensed Practical Nurse to provide hands-on care in our skilled nursing and rehab facility. The LPN will deliver direct resident care, administer medications and treatments, and monitor clinical status. Responsibilities include supporting ADLs, documenting accurately, and communicating changes to the RN and provider. You'll collaborate with an interdisciplinary team, supervise CNAs, and participate in care planning and quality initiatives.

Responsibilities

• Provide direct nursing care to residents including medication administration, treatments, and monitoring vital signs
• Assist with assessments and documentation of residents' conditions, reporting changes to the RN and physician
• Support residents with daily living activities while promoting dignity, comfort, and safety
• Collaborate with interdisciplinary team members to implement and update individualized care plans
• Educate residents and families on care plans, treatments, and health maintenance
• Maintain accurate, timely clinical records and follow all regulatory and facility policies
• Participate in quality improvement, infection control, and safety initiatives
• Supervise and support CNAs and other care staff to ensure consistent, high-quality care
