PAY RATE: $24.85#-#$37.28 BASED ON RELEVANT EXPERIENCE COMPETITIVE BENEFITS #to review our complete Total Rewards Guide.# Retirement + matching Up to 4 weeks paid time off in first year Onsite childcare -#Quincy# 24/7 Wellness Center access Educational assistance opportunities JOB SUMMARY The Discharge Planner partners with the Unit C3, nursing, Social Worker and providers to ensure a discharge plan is created and keeps the care team, patient and family updated on the discharge plan.

It is their responsibility to be knowledgeable of discharge and pay specific utilization criteria in order to ensure patients# needs are fully met with resources at the time of discharge. The Discharge Planner will also be aware of the continuum of care beyond the patient#s inpatient stay. This position requires full understanding and active participation in fulfilling the Missions of Blessing Health Systems.
