* About the Role:

** As a LPN supporting Nursing Home Services at Quincy Medical Group, you will play an important role in coordinating and delivering care for patients residing in long-term care and skilled nursing facilities. Working closely with providers, facility staff, patients, and families, you will help ensure continuity of care, timely communication, and exceptional clinical support. This position is ideal for a nurse who enjoys building relationships, working independently, and serving a vulnerable patient population.
** Schedule & Work Environment
*** Full-time position, Monday-Friday, with a typical schedule of 8:00 AM – 5:00 PM.
* Schedule flexibility may be available based on departmental needs.
* Position based in Quincy, Illinois.
* Collaborative environment working closely with providers, facility staff, patients, and families.
** Primary Responsibilities
*** Support providers in delivering care to residents in nursing home and long-term care settings.
* Coordinate patient care activities, including follow-up appointments, referrals, testing, and treatment plans.
* Communicate with nursing facility staff regarding provider orders, patient concerns, and changes in condition.
* Assist with patient assessments, medication management, and care coordination activities within scope of practice.
* Review and respond to clinical communications from facilities, patients, families, and providers.
* Maintain accurate and timely documentation within the electronic medical record.
* Monitor and track laboratory results, diagnostic testing, and follow-up recommendations.
* Provide patient and family education regarding treatment plans and ongoing care needs.
* Collaborate with interdisciplinary teams to support quality patient outcomes and continuity of care.
* Perform other duties as assigned to support nursing home services operations.
** Qualifications
* ** Current Illinois Licensed Practical Nurse (LPN) license required.
* Current CPR certification required.
* Previous nursing experience in long-term care, skilled nursing, geriatrics, primary care, or care coordination preferred.
* Strong assessment, communication, and organizational skills.
* Ability to work independently while managing multiple priorities.
* Comfort communicating with patients, families, providers, and facility staff.
* Experience with electronic medical records preferred.
* Commitment to providing compassionate, patient-centered care.

The compensation for this role includes a base pay range of **$18.66 - $27.99 per hour**, with actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
