AboutThe Role
As an RN Care Manager in Orthopedics at Quincy Medical Group, you will combine hands‑on clinical nursing with care management to help patients successfully navigate their orthopedic care. Working closely with orthopedic providers, patients, families, and other members of the care team, you will coordinate services, identify barriers to care, provide patient education, and support patients throughout treatment and recovery.
In addition to care management responsibilities, you will provide traditional clinic nursing support based on patient and department needs. This role is ideal for an experienced nurse who enjoys developing relationships with patients, coordinating complex care, and remaining actively involved in direct clinical practice.
Schedule & Work Environment- Full-time position, Monday-Thursday, 7:00am - 5:00pm, Friday 7:00am - 1:00pm.
- Schedule may vary based on provider, patient, and departmental needs.
- Fast‑paced outpatient orthopedic environment combining care management, patient education, and direct clinical nursing responsibilities.
- Provide care management services for orthopedic patients, coordinating care across providers, specialties, therapy, diagnostic services, and other resources.
- Assess patient needs and identify clinical, social, or logistical barriers that may affect treatment, recovery, or follow‑up care.
- Develop and support individualized care plans in collaboration with patients, families, providers, and the interdisciplinary care team.
- Educate patients and caregivers regarding diagnoses, treatment plans, medications, procedures, recovery expectations, and follow‑up needs.
- Monitor patient progress and coordinate appropriate follow‑up to promote continuity of care and positive outcomes.
- Provide direct clinical nursing support, including patient assessment, medication management, telephone triage, care coordination, and assistance with procedures within scope of practice.
- Review patient messages, test results, medication requests, and other clinical needs and communicate with providers as appropriate.
- Coordinate referrals, imaging, laboratory testing, procedures, and other services needed throughout the patient’s orthopedic care.
- Maintain timely and accurate documentation within the electronic medical record.
- Collaborate with orthopedic providers and other members of the care team to improve patient access, clinical workflows, and continuity of care.
- Serve as a resource and advocate for patients and families throughout their orthopedic treatment and recovery.
- Perform other duties as assigned to support Orthopedic operations.
- Current Illinois Registered Nurse (RN) license required.
- Graduate of an accredited nursing program required.
- Current CPR/BLS certification required.
- Previous experience in orthopedics, ambulatory nursing, care management, case management, or a related clinical setting preferred.
- Strong clinical assessment, critical thinking, care coordination, and patient education skills.
- Ability to independently prioritize patient needs while collaborating effectively with providers and multidisciplinary care teams.
- Excellent communication, organization, and relationship‑building skills.
- Experience with electronic medical records preferred.
