Licensed Practical Nurse

Americare Senior Living seeks a compassionate Licensed Practical Nurse to provide resident-centered care in our senior living community. In this role, you will administer medications, perform treatments, monitor vital signs, and support residents with daily needs while following individualized care plans. You'll collaborate closely with RNs, CNAs, physicians, residents, and families to promote dignity, safety, and comfort. Responsibilities include accurate documentation, responding to changes in condition, supporting memory care needs, and mentoring support staff within our respectful, team-focused environment.

Responsibilities

• Provide resident-centered nursing care to seniors in assisted living and skilled settings
• Administer medications and treatments per physician orders and facility policy
• Monitor and document vital signs, conditions, and responses to care in EHRSupport residents with ADLs while promoting independence and dignity
• Collaborate with RNs, CNAs, physicians, and families on individualized care plans
• Recognize and report changes in resident condition and respond to emergencies
• Follow infection control, safety, and regulatory standards at all times
• Educate residents and families on health, medications, and care plans
• Assist with memory care and behavioral support for residents with dementia
• Mentor and support CNAs and new team members to ensure quality care
