Patient Placement Coordinator

• Manage and coordinate patient placement from external and internal sources
• Facilitate collaboration between departments and healthcare providers for timely admissions
• Conduct documentation and tracking in electronic medical records
• Monitor patient flow and bed availability to ensure efficient operations
• Communicate with referring sites and provide necessary information for patient transfers

• Associate's or Vocational degree
• 2 to 5 years of experience in the healthcare field, including familiarity with medical terminology
• 2 to 5 years of computer skills
• Ability to work evening shifts and every other weekend
• Strong communication and coordination skills

• Bachelor's degree
• Experience with Epic / Excellian electronic medical records
• Previous experience in patient placement or hospital admissions
