The RN Field Clinical Care Coordinator works as part of an Integrated Care Model by relaying pertinent information about members' needs, advocating for the best possible care, and ensuring members have access to the right services.

If you reside in the Braintree, Weymouth, Quincy, MA market and surrounding areas, you may have the flexibility to work telecommute as you take on challenges. This is a field-based role with at least 50% of time spent in the field visiting members in their homes.

You will need to be flexible, adaptable and patient in all types of situations.

Primary Responsibilities
  • Engage members face-to-face and/or by telephone to complete a comprehensive needs assessment, including medical, behavioral, functional, cultural, and socioeconomic needs
  • Assess, plan and implement care strategies individualized for patients and directed toward the most appropriate, least restrictive level of care
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
  • Manage the care plan throughout the continuum of care as a single point of contact
  • Partner and collaborate with the internal care team, providers, and community resources/partners to implement the care plan
  • Communicate with all stakeholders the required health-related information to ensure quality, coordinated care and services are provided expeditiously

Required Qualifications
  • Current and unrestricted licensure as a Registered Nurse in the state of MA
  • 2+ years of clinical experience
  • Intermediate proficiency with MS Office (Word, Excel, Outlook)
  • Access to a designated quiet workspace at home and ability to secure PHI
  • Residence in an area with United Health Group approved high-speed internet or access to high-speed internet
  • Ability to travel in assigned regions to visit members in homes and/or other settings
  • Access to reliable transportation and valid US driver’s license
Preferred Qualifications
  • Bachelor's or master's degree in nursing
  • Certified Care Manager (CCM)
  • 1+ years of community case management experience coordinating care for individuals with complex needs
  • Experience working in team-based care
  • Background in Managed Care
  • Experience using an Electronic Medical Record or similar electronic platforms
  • Bilingual in Cantonese, Mandarin, or Vietnamese
Physical Requirements
  • Ability to transition between office and field locations multiple times per day
  • Ability to navigate multiple locations and terrains to visit members, employees and/or providers
  • Ability to transport equipment to field locations (e.g., laptop)
  • Ability to remain stationary for long periods to perform computer or tablet work

Pay is based on several factors including location, education, experience, and certifications. The role offers a comprehensive benefits package; the hourly pay range is $28.94 to $51.83 based on full-time employment. We comply with all applicable minimum wage laws.
