• Provide post‑acute LPN nursing care to residents with varying medical needs
• Track vital signs, physical condition, and mental acuity of residents
• Complete intakes of residents, documenting medical histories and health concerns
• Assist with medications, including proper administration, verification, and monitoring
• Maintain accurate medical records and charts
• Help maintain a safe, comfortable, and pleasant environment for residents
• Ensure proper storage and inventory of medical supplies
• Set up and assist with immunizations, IVs, and phlebotomy procedures
• Follow and adhere to care plans for residents and all applicable policies and regulations
• Support training and orientation of new clinicians

• Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
• Completion of an approved LPN educational program
• High school diploma or GED (or equivalent)
• At least 6 months experience as an LPN or similar role
• Ability to lift 50 lbs
