• Support the diverse needs of residents in post-acute facilities
• Monitor residents’ mental and physical conditions and vital signs
• Facilitate the intake process, including documentation of medical histories and health issues
• Assist with medication administration, verification, and monitoring of effects on residents
• Maintain residents’ medical records and charts
• Help with immunizations, IVs, and blood draws
• Ensure residents have a comfortable and safe environment
• Adhere to residents’ care plans and comply with all applicable rules and regulations
• Contribute to the maintenance and proper inventory of medical supplies
• Assist with new hire orientation and training

• Current Ohio state license to practice as a Licensed Practical Nurse (LPN)
• Completion of an approved Licensed Practical Nurse (LPN) educational program
• High school diploma or GED (or equivalent)
• Minimum of 6 months experience as an LPN (or similar role)
• Ability to lift 50 lbs
