Discovery Senior Living - 99 Brackett Street - Responsibilities:
Provide care and support to a designated group of residents during your assigned shift, ensuring awareness of resident location and well-being.;
Observe, document, and report changes in resident condition including skin integrity, behavior, alertness, weight, dietary intake, and participation in activities.;
Respond promptly to resident call systems, alarms, and safety alerts.;
Communicate changes in care needs to supervisors or the Health Care Coordinator.;
Assist with continence management while maintaining sanitation of resident suites and community restrooms.
