Volunteers of America Massachusetts seeks a Community Support Navigator to provide person-centered support to individuals and families accessing housing, behavioral health, reentry, veterans, and supportive services. In this role, you will complete intakes and assessments, develop individualized service plans, and connect clients to internal programs and community resources that promote stability, recovery, and independence. You will coordinate care across providers, maintain accurate documentation, and advocate for equitable access to services.

Working in a collaborative, mission-driven environment, you will help advance our commitment to social justice, inclusion, and client-centered outcomes while gaining experience in nonprofit human services.

Responsibilities

  • Provide direct support, advocacy, and navigation assistance to individuals and families accessing housing, behavioral health, reentry, and veterans' services
  • Conduct client intake, needs assessment, and individualized service planning focused on stability, recovery, and independence
  • Connect clients to internal and external community resources, benefits, and referrals; coordinate care across providers
  • Maintain accurate documentation, case notes, and data in compliance with agency and funder requirements
  • Collaborate with clinical, housing, and reentry teams to support client-centered care and successful transitions
  • Facilitate groups, workshops, or orientations that build life skills and increase access to services
  • Monitor client progress, adjust service plans, and support crisis intervention or de-escalation as needed
  • Advocate for clients' rights and access to equitable services across systems and community partners
  • Participate in team meetings, case conferences, and trainings to support best practices and continuous improvement
  • Uphold Volunteers of America Massachusetts' mission, values, and cultural commitment to equity, inclusion, and social justice
