RN Care Manager - Full-Time - Day Shift

Community Care is hiring a RN Care Manager to join our PACE Program team. This is a full-time opportunity at our Racine location.

RN Care Manager

Job Responsibilities:

Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed.

• Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.
• Complete all required documentation and charting and assists in the development and maintenance of member records.

Job Requirements:

Education:

Associates degree in Nursing Required, Bachelors Degree preferred.

Licensure:
Wisconsin Licensed Registered Nurse

Licensure:
Eligibility for long-term care functional screener certification is required

Licensure: WI Drivers License

Experience:

Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity. Preferred 2 years of work in community nursing.
