Job Title
Hospice Admission Registered Nurse / RN
Role OverviewCompassus is a home health and hospice organization committed to caring for patients and employees. This position is a Hospice Admission Registered Nurse whose primary duty is to assess hospice eligibility and coordinate initial care plans.
Responsibilities- Enable patients to spend quality time with their loved ones and engage in activities they enjoy
- Assess patients for hospice eligibility and explain hospice care services to patients, families, and caregivers
- Ensure all patient’s initial needs are addressed
- Collaborate with the interdisciplinary team members regarding initial plan of care, patient’s condition, and other essential information
- RN license in the state you work
- Two years of experience as a registered nurse; hospice experience preferred
- Current driver’s license
- Current CPR and BCLS certification
