Position Overview
As the Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care to acutely and/or chronically ill patients in their homes, record clinical and progress notes, and coordinate patient care activities/services to deliver high‑quality care across the geographic service area assigned.
Primary Responsibilities- Provide high‑quality clinical services within the scope of practice defined by state nursing laws, in accordance with the plan of care, and in coordination with the health care team, following all relevant infection control procedures.
- Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record, using the agency’s electronic visit verification program as applicable.
- Participate actively in the agency’s care coordination process, including timely reporting of patients’ needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, case coordinator, and community case managers as applicable.
- Function in any home setting regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
- Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
- Current driver’s license and vehicle insurance, access to a dependable vehicle, or use of public transportation.
- Current CPR certification or ability to obtain within 90 days of hire.
- Ability to work flexible hours as required to meet identified client needs.
- Ability to manage multiple tasks simultaneously.
- Ability to work independently.
- Strong communication, writing, and organizational skills.
Pay is hourly, ranging from $34.05 to $51.10 based on full‑time employment, and based on local labor markets, education, work experience, and certifications.
