Role: Hospice RN providing end-of-life skilled nursing care in patients' homes and facilities.
Responsibilities Include- Assessing patient needs, developing and coordinating individualized care plans with the healthcare team.
- Performing tasks such as vitals, wound care, IV, ostomy care, blood draws, and injections.
- Collaborating with physicians and multidisciplinary teams, documenting visits and care plans accurately.
- Managing discharge planning, providing emotional and spiritual support, and responding to patient and family calls.
- Ensuring compliance with policies, procedures, and regulations of home and hospice care.
- Graduate of an accredited nursing program or equivalent.
- Valid RN license in applicable states; BLS certification.
- Strong communication skills, alertness, and ability to work independently.
- Physical ability to perform patient care tasks, including lifting and mobility assistance.
Valid driver’s license, insurance, and reliable transportation.
Key dutiesProviding compassionate end-of-life care, ensuring safety, accurate documentation, and effective communication with patients, families, and healthcare team.
