POSITION PURPOSE

The Intensive Outreach Specialist is responsible for helping reduce group and retaliatory violence in the City of Racine by working directly with adults who are identified as very high-risk of being involved in gun violence. The Intensive Outreach Specialist is tasked with establishing trusting relationships with these adults by spending significant time with their participants, helping them reduce their risk factors, and connecting them to services, supports, and opportunities.

The Intensive Outreach Specialists reports directly to Director of the Department of Community Safety.

ESSENTIAL DUTIES
  • Reduce retaliatory group violence in the City of Racine.
  • Supportive Outreach and Case Management with High-Risk Individuals
    • Conduct outreach in the community to connect with individuals identified as very high risk of being victims or perpetrators of gun violence.
    • Participate in custom notifications of high-risk adults and enroll in intensive case management services.
    • Initiate contact via any means of communication with referred adults within 24-hours after assignment from the supervisor.
    • Initiate an in-person contact with referred adults within 72 hours of assignment from the supervisor.
    • Establish a positive and trusting relationship with each assigned participant.
    • Develop a Comprehensive Case Plan with each assigned participant.
    • Connect participants to services as appropriate, based on self-determined and assessed needs.
    • Use relevant personal experiences and established relationships to influence high-risk adults’ decision-making processes to improve their outcomes.
    • Maintain consistent contact with each participant on the case load in alignment with their phase:
      • Phase One:
        First 1-3 months on case load:
        Daily communication, 3x/week in-person contact
      • Phase Two: 3-6 months on case load:
        Minimum 3x/week communication, 1x per week in-person contact
      • Phase Three: 6-18 months on case load:
        Minimum 1x/week communication, as needed in person contact
  • Victim/Survivor Support and Outreach and Relocation
    • Establish a positive and trusting relationship with each assigned participant/family.
    • Assess and evaluate immediate needs
    • Advocated for and connect participants with services and resources as appropriate, based on self-determined and assessed needs.
  • Document case management efforts, update individual case plans, and make all required entries into the data management system.
  • Attend all mandatory and necessary meetings and trainings, including team meetings, conferences, and all other meetings and trainings.
  • Follow all directions and guidance from your supervisor.
  • All other assigned duties.
ENVIRONMENTAL / WORKING CONDITIONS OF THE POSITION

Office and a wide variety of indoor/outdoor environments.

MINIMUM QUALIFICATIONS
  • Minimum of a High School diploma, HSED, GED or equivalent
  • Experience working with high-risk youth or adults
  • A Valid driver’s license at time of appointment and throughout employment
PREFERRED CANDIDATES WILL HAVE
  • Direct lived experience with gang/group involvement or street violence
    • Demonstrated history of disassociation from involvement in gang/group involvement and activity, and street violence and crime
  • Legitimacy and relationships with those who are still actively gang/group involved, or their relatives
KNOWLEDGE, SKILLS, QUALIFICATIONS, AND ABILITIES REQUIRED
  • Oral communication skills to effectively communicate with diverse individuals who have:
    • extensive criminal justice involvement.
    • are members of or associated with gangs/groups.
    • have been shot at or shot before, and/or have close friends/relatives who have been shot recently.
  • Knowledge of or willingness to learn about the public health approach to violence prevention.
