ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Establish and maintain effective working relationships with clients, families, caregivers, service providers, volunteers, community agencies and the public. Promote positive communication and support within the screen team and provide consultation with Connections IRIS consultants regarding LTCFS procedures and budgets.
  • Administer LTCFS annually and/or when rescreens are requested.
  • Coordinate and collect data for LTCFS renewals.
  • Provide highly effective support and assistance to enable individuals to understand and meet requirements to obtain and maintain funding for support and services through IRIS.
  • Continuously seek to learn and share better ways to enable those we serve to lead more fulfilling lives.
  • Work as part of an ICA team to participate in peer mentoring, learning, and mutual support to others within the team.
  • Participate in staff meetings and collaborate regarding eligibility/ineligibility for a rescreen.
  • Strengthen connections within neighborhoods and communities that may foster mutual support and caring among individuals served, their neighbors, and other citizens.
  • Maintain case records in compliance with agency, state, and federal standards.
  • Prepare written reports and may testify in court during evidentiary hearings, contested cases, and annual court reviews.
  • Ensure proper administration of required documentation for federal and state funding sources.
  • Consult and coordinate with partnering agencies such as Self‑Directed Personal Care (SDPC) and Medical Assistance Personal Care (MAPC) agencies.
  • Perform record‑keeping including regional LTCFS spreadsheets, referral tracking sheets, verification of vital information, and screen data.
  • Participate in training and mentoring of new screeners.
  • Coordinate Long‑Term Care Functional Screen (LTCFS) team liaison responsibilities with the Department of Health Services (DHS), IRIS consultants, and other related agencies to establish and maintain high‑quality screening services.
  • Maintain a valid Certification of a Long‑Term Care Functional Screen.
QUALIFICATIONS
  • Bachelor’s Degree in social work, psychology, human services, counseling, nursing, special education, or an equivalent combination of education is required.
  • Minimum of one year of experience completing LTCFS with seniors and/or people living with disabilities is preferred.
  • Must have a valid driver’s license, auto insurance, and reliable transportation to perform essential duties.
  • Strong commitment to enabling people to be in control of their lives.
  • Strong commitment to supporting people’s opportunities to live meaningful lives as valued citizens, friends, and neighbors.
  • Capacity to support individuals served to meet required IRIS policies and procedures in ways that maximize administrative ease.
  • Capacity to work independently and as part of a creative, flexible, empowered ICA/Functional Screen Team.
  • Capacity to use and apply smartphone, tablet, and related technology.
  • Considerable knowledge of human behavior, dynamics of groups, interpersonal relations, negotiation, and social interaction.
  • Considerable knowledge of social problems and their effects on individuals.
  • Ability to organize and plan work effectively.
  • Ability to establish and maintain effective public and working relationships.
