• Resolve department issues and implement organizational initiatives for the team.
• Maintain clinical competencies and provide professional care to patients of all ages.
• Assess, plan, implement, and evaluate patient care to achieve optimum outcomes.
• Lead the team to achieve positive patient outcomes.
• Follow up on matters to ensure successful resolution.
• Assist leadership in ensuring staff competence and contribute to staff performance development.
• Identify areas of improvement, provide input, and work with employees to ensure success.
• Maintain performance-related information and participate in annual reviews.
• Conduct individual case conferences and participate in interdisciplinary team meetings for effective communication.
• Mentor and coach employees; provide performance feedback.
• Listen to staff needs and concerns and provide resolutions.
• Follow up with staff on identified issues and report to leadership.
• Coordinate workload for the home care/hospice team.
• Maintain adequate staffing levels, monitor work hours, and perform patient visits as necessary.
• Handle clinical issues for the agency and escalate to leadership as needed.
• Monitor staff productivity, mileage, and supply usage for proper resource allocation and expense control.
• Collaborate with leadership on fiscal management.
• Assist with other agency responsibilities, including regulatory compliance, documentation review, ICD‑10 coding, re-certifications, performance improvement, and quality improvement initiatives.
• Ensure Joint Commission, Medicare, and other third‑party payer guidelines are implemented and followed.
• Maintain compliance with policies and procedures.
• Promote a positive work environment by holding staff accountable and using effective interpersonal skills.

• Education:
  
  Graduate of an accredited Registered Nurse program.
• Experience:
  
  5 years of clinical experience.
• Licensure:
  Current RN license in Virginia or an approved compact state.
• Vehicle:
  Valid driver’s license, automobile insurance, and personal vehicle for home visits.
• Life Support:
  None required.
• Leadership:
  Demonstrates leadership qualities, strong interpersonal and communication skills, and team‑building ability.
• Computer:
  Keyboarding skills required for electronic medical records and automated systems.
• Knowledge:
  Understanding of life‑span growth and development, end‑of‑life care, and ability to identify patient needs based on age.
• Core Competence:
  Ability to maintain professional care standards and support staff performance.
